You are required to generate an invoice, which would then be taken to any of the designated banks. See full application guide below.

• Go to www.fpno.edu.ng to purchase the screening form for a fee of N2,000
• After payment proceed to ndapplication.fpno.edu.ng/Home/Form

The following documents/information are required while filling the form:

• O’level Result (WASSCE, NECO & NABTEB).
• 2019/2020 UTME Score.
• JAMB subject combination for the course applied.
